Post-Baccalaureate Program Application

General Guidelines

The application opens November 15 and closes at 11:59pm on January 15. Late application materials are not accepted, period.

Follow all instructions/guidelines for the submission of application materials. Materials received in any other way will not be accepted and the application will be considered incomplete.

Specific instructions for each application section can also be found throughout the online application.

We will not be reviewing applications until the January 15 deadline. We will notify you about 2 weeks after the deadline to let you know whether you've received an interview.

The Online Application

Do not wait until the last minute to submit this. Electronic glitches will not be accepted as an excuse for late submission.

Read instructions thoroughly, including answer formatting requests.

You can save and return to your application by creating a free account with FormSite. This is recommended so you don't have to complete it all at once, or start over. Be sure to make note of your login information and return link.

The Personal Statement

  • Single-spaced, 11-12pt font, and about 1 to 1 ½ pages in length.
  • Make sure your full name is in the header of your paper.
  • Be sure to include: why you want to be a physician, and why you are pursuing the Post-Baccalaureate Program.
  • The purpose of the personal statement is to assess your motivation, communication style, and learn more about who you are (not a list of what you've done - which we can get out of your online application.)
  • Save and submit the statement as a PDF using the upload tool in the online application.

Letters of Recommendation

Print 3 "Post-Baccalaureate Program Reference Request Forms" available from a link in the online application. Provide this form for each of your letter writers. They will need to include it when they submit their letters for you.

Three letters of recommendation are required. We request that one be from a clinical environment, one from an academic environment, and one "other" such as a boss or community leader you've worked with. (Note: committee letters will be considered 1 academic letter.) No more than 3 letters will be accepted.

It is a good idea to request letters early and in person. Provide your letter writers with a current resume and information about the Post-Bac Program (see next page.)

Letters must be submitted electronically and directly to the Post-Baccalaureate Program. Submission via Interfolio is also acceptable.


You must submit transcripts from ALL undergraduate institutions where you have completed coursework. This includes all community colleges as well.

Request transcripts early. We must receive them by January 15.

Transcripts must be official and sent directly to the Post-Baccalaureate Program from the institution at:

You will be asked to upload a color photo of yourself at the end of the application. JPG, PNG, or PDF are preferred.

A headshot is preferred. We recommend that you are dressed professionally for the photo.
