This FREE program will

  1. Address two key health issues and the unique needs of the elderly re-entry population
  2. Provide health care professionals with tools and additional resources to tackle diabetes management and recurrence of substance abuse to promote healthy transitioning into the community
  3. Discuss ways of improving the quality of elder re-entry adults into the community with respect to diabetes and substance abuse


Panelists include: Richard Davis, Community Health Worker at Dignity Health; St. Rose Dominican Dani Tillman, MSW, LSW, LADC, Chief Operating Officer at Ridge House, Inc.
