Department of Family and Community Medicine

Successful applicants must qualify for Board Certification in Family Medicine and licensure in the state of Nevada. For the application process, please visit the American Medical Society for Sports Medicine.

To be considered for the fellowship program, applicants must submit:

•  Letter of interest
•  Medical school transcripts
•  Copies of USMLE scores
•  AMSSM Code of Ethics
•  Three letters of recommendation (One of the letters of recommendation must be from the applicant's residency Program Director).

All application materials must be received no later than November 1 through the ERAS database.
