Malpractice verification instructions

If a hospital or agency requires insurance verification and/or claims history information, please have the hospital and/or agency send these requests to the General Counsel's Office, via fax at (775) 327-2009. If these requests are sent directly to ProAssurance (our carrier), this will result in the response being delayed for up to a week. Please be sure that these requests are accompanied by an authorization that the provider has signed and dated giving permission for ProAssurance to release this information to the hospital and/or agency. Note, digital signatures will not be accepted. If there has been a change in name since coverage dates, please note the change on the request in order to expedite the verification process.

Malpractice Claim

If you receive a lawsuit naming you as a defendant, a written communication alleging malpractice and/or demanding a settlement, or a call from an attorney representing a patient, please immediately refer the matter to Matthew Milone, Senior Associate Dean for Legal Affairs.  Please contact Mr. Milone via email at mmilone@med.unr.edu including: your name, your email, the best phone number to reach you, the name of the patient, the date(s) of service.  Please also attach a copy of any correspondence you received.  Do not respond to a letter or speak to an attorney representing a patient without first consulting with Mr. Milone.  

Board Complaint

If you receive a letter from your licensing board requesting information or a written response, please immediately refer the matter to Matthew Milone, Senior Associate Dean for Legal Affairs.  Please contact Mr. Milone via email at mmilone@med.unr.edu including: your name, your email, the best phone number to reach you, the name of the patient, the date(s) of service.  Please also attach a copy of the letter from the Board.  Do not respond to the Board without first consulting with Mr. Milone.

Subpoena to Testify

If you receive a subpoena to testify in a case relating to your employment with UNR Med or care provided through UNR Med's practice plan, please immediately refer the matter to Matthew Milone, Senior Associate Dean for Legal Affairs.  Please contact Mr. Milone via email at mmilone@med.unr.edu including: your name, your email, the best phone number to reach you, and dates that you would be available to testify.  Please also attach a copy of the subpoena.  Do not respond to the subpoena or speak to the attorney issuing the subpoena without first consulting with Mr. Milone.
