The Online BS-MD Application

  1. Do not wait until the last minute to submit this. Electronic glitches will not be accepted as an excuse for late submission.
  2. Read instructions thoroughly, including formatting requests.
  3. You can save and return to your application by creating a free account with FormSite. This is recommended so you don't have to complete it all at once, or start over. Be sure to make note of your login information and return link.

Supplemental Application Questions

In lieu of a personal statement, you will be asked to respond to 4 supplemental questions in the application. Be sure to take you time in crafting your answers, and also be sure that your responses are true to YOU.

  1. You will be required to have 3 people complete the online recommendation form for you. Recommendations must be from each of the following:
    1. one math or science teacher
    2. one from any academic environment
    3. one from a community or clinical environment such as a physician you shadowed, a volunteer coordinator, boss or community leader with whom you've worked
  2. Note that as soon as you save your recommenders contact information into the online applicatoin, it will automatically email a recommendation form to your contacts. We highly recommned that you ask your recommenders ahead of time if they are willing to submit on your behalf so as not to catch them off-guard.
  3. We recommend providing your recommenders with information about the BS-MD Direct-Entry Program.

You must submit transcripts from all high schools where you have completed coursework and any colleges where you may have already completed coursework as well.

Request transcripts early. We must receive them by January 22.
