Patient-centered family medicine is an innovative program for improving primary care. We focus on organizing medical care around you, working in teams and coordinating and tracking care over time.

Our primary goal is to give you the best possible medical care by building a trusting partnership between you, as an informed patient, and the health care team. The center of the team is you, the patient.

Our Responsibilities

As your health care providers, we will respect you as an individual, listen to your feelings and answer your questions to help you make decisions and set healthy goals.

We will provide the best possible treatment and advice based on current medical evidence. To do this, we must have information, including a list of your medications, health status, recent test results, visits to specialists and data from recent hospitalizations or visits to the emergency room.

We will end every visit with clear instructions about expectations, treatment goals, medicines and future plans, including 24-hour phone access to your health care team. All of your medical information and records will be kept private.

Patient Responsibilities

In order for us to provide you with the best possible care, we ask that patients learn about wellness and preventing disease in order to make healthy decisions. It's important that patients learn what their insurance covers and be honest and thorough about medical history, symptoms and changes in health.

Telling us what medications you take and asking for refills during office visits, when you see other doctors, when medications have been prescribed, and asking them to send us a report about your care is also important.
