PGY-4 Twelve Month Rotation at Life Change Center

By the end of the rotation the resident should:

Patient Care

  1. Demonstrate the ability to apply the criteria for inclusion in opiate maintenance programs by successfully admitting 5 patients for Methadone maintenance therapy.
  2. Demonstrate the ability to apply the principals of opiate dosing by successfully stabilizing 5 patients on methadone or suboxone.
  3. Demonstrate the ability safely detoxify patients from opiates by successfully initiating detoxification protocols on at least 5 patients

Medical Knowledge

  1. Know the signs and symptoms of common medical problems seen with opiate addiction as demonstrated by at least 5 complete history and physical examinations.
  2. Know the common psychiatric manifestations of opiate addiction as demonstrated by at least five complete psychiatric evaluations.
  3. Know the signs and symptoms of opiate withdrawal as demonstrated by at least 5 evaluations of patients for signs and symptoms of withdrawal.
  4. Describe the risks and side effects of opiate maintenance induction as demonstrated by observed discussion with patients entering the program.
  5. Know the criteria for detoxification as demonstrated by identifying 5 patients suitable for detoxification.

Practice Based Learning and Improvement

  1. Demonstrate the ability to find appropriate education material for self education and patient needs as determined by their supervisor.

Interpersonal and Communication Skills

  1. Demonstrate the ability to effectively communicate with patients concerning sensitive issues such as drug use, and its impact on family, legal system, and physical health as evidenced by direct observation of staff and supervisor.

Professionalism

  1. Demonstrate the ability to act as a patient advocate concerning Opiate Maintenance Therapy by interacting when necessary with other health care providers, organizations, employers and families.

Systems-Based Practice

  1. Demonstrate adequate knowledge of the various regulatory agencies and rules which govern Opiate Maintenance Therapy providers as evidenced on interview with the clinic’s executive director.
