We offer programs and services for elders and their care partners that are designed to complement care they receive from their own primary care provider and other specialists.

Comprehensive Geriatric Assessment

Our "whole person" assessment is designed to evaluate physical, cognitive, emotional, and social health as well as other key dimensions of well-being. The 90-minute appointment includes:

  • Personal health history and review
  • Physical health assessment Review your risk for falls/frailty, memory and/or cognition issues, activities of daily living (dressing, eating, hygiene, etc.), and mobility. Note: This is not a physical examination. You will remain fully clothed.
  • Behavioral health/psychosocial assessment Social support/family issues, housing, financial concerns, transportation and any other concerns identified by the patient.
  • Medication therapy management  Review of all prescriptions, over-the-counter medications, and dietary supplements; and specific recommendations. Be sure to bring all of your medications with you.
  • Personalized care plan A customized plan to share with your other care providers, including advance directives.
  • Referrals to additional services or providers, if needed

Specialty Geriatric Care Services

  • Chronic Care Management

    For clients who are dealing with ongoing health conditions, our chronic care management service can help coordinate home and clinical services, facilitate communication with multiple care providers, and manage care transitions as needed.

  • Medicare Annual Wellness Visit
    • Available to all Medicare Part B enrollees, this one hour appointment is designed to help prevent disease and disability based on your current health and risk factors. It includes:
    • A review of your medical and family history
    • Developing or updating a list of current providers and prescriptions
    • Height, weight, blood pressure, and other routine measurements
    • Detection of any cognitive impairment
    • Personalized health advice
    • A list of risk factors and treatment options for you
    • A screening schedule (like a checklist) for appropriate preventive services. Get details about coverage for screenings, shots, and other preventive services.
    • Advance care planning
  • Social Work Counseling/Therapy
    • For patients who want help working through complex emotions, health care transitions, managing issues around chronic health conditions or finding new sources of enjoyment and social support, our licensed clinical social worker provides one-on-one counseling and therapy for patients and their care partners.
  • Complement to Primary Care

    As a specialty service only, we do not provide primary care services. For treatment of illnesses, injuries, diseases or other symptoms, we recommend you consult a primary care physician.
