We are committed to the success of your medical treatment and care. Thank you for choosing our practice! For your convenience, we have answered a variety of commonly-asked financial policy questions below.   • What is my financial responsibility for services?

    Your financial responsibility depends on a variety of factors.

  • How may I pay?

    We accept payment by cash, check, VISA, MasterCard, and American Express.

  • Do I need a referral?

    We will make every effort to obtain the necessary referral information prior to your visit. If you have an HMO plan with which we are contracted, you will need a referral authorization from your primary care physician. If we have not received an authorization prior to your arrival at the office, we have a telephone available for you to call your primary care physician to obtain it. If you are unable to obtain the referral at that time, you will be rescheduled.

  • What if my child needs to see a physician?

    A parent or legal guardian must accompany patients who are minors on the patient’s first visit. This accompanying adult is responsible for payment of the account, as outlined above, at the time of your visit.

  • No show appointment

    You are required to call our office to cancel 24 hours prior to your appointment. This allows other patients who are in need of a doctor’s care an available appointment. If you fail to cancel 24 hours prior to your appointment you will be assessed a $25.00 fee which will be added to your account and must be paid in full prior to any follow up visits. If you fail to show up for a scheduled appointment more than twice during a one year period, you may be asked to seek care from another provider.

Commercial Insurance
Also known as indemnity, “regular” insurance, or “80/20 coverage” or; HMO & PPO plans with which we have a contract
If the services you receive are covered by the plan: All applicable co-pays and deductibles are required at the time of your visit. If the services you receive are not covered by the plan: Payment in full is required at the time of your visit. Check to see if your insurance is contracted with us. We will also call your insurance company ahead of time to determine deductibles and coinsurance. We will fine an insurance claim as a courtesy to you.
Commercial Insurance or; HMO/PPO with which we are not contracted Payment in full is required for office visits, x-rays, injections, and other charges at the time of your visit. File one claim on your behalf; however, if no payment is received the account will be re-classified as “self pay”.
Medicare If you have Regular Medicare, and have no met your $110 annual deductible, we require that your deductible be paid at the time of service. Payment for services not covered by Medicare is required at the time of your visit. If you have Regular Medicare as primary, and also have secondary insurance or Medigap: No payment is necessary at the time of your visit. If you have Regular Medicare as primary, but no secondary insurance: Payment of your 20% co-pay is required at the time of your visit. File the claim on your behalf, as well as any claims to your secondary insurance.
Medicare HMO Payment of all applicable co-pays and deductibles are required at the time of your visit. File the claim on your behalf, as well as any claims to your secondary insurance.
Medicaid You must have a valid card in hand at the time of service or your appointment will be rescheduled. Verify your eligibility and file the claim on your behalf.
Secondary Insurance Payment of all applicable co-pays and deductibles are required at the time of your visit. File one claim on your behalf; however, if no payment is received the account will be re-classified as “self pay”.
No Insurance You are considered a “self pay” patient and payment in full is required at the time of your visit. If you are unable to pay in full for your care, we have programs available to assist you:
  • 20% discount on charges if payment is received in full at the time of your visit.
  • Alternative financing – 15% APR (AmericanWest Bank)
  • If your annual gross income is less than $25,000 you may qualify for financial aid including discounted patient care or write off.
