Specimen Identification Label slide with patient’s first name, last name and DOB (use pencil).
Special Instructions
  • Indicate specimen source on requisition slip
  • Submit 1 smear
  • DO NOT allow the smears to dry before applying the fixative provided in the Pap-Pak™ when the sample is collected.
  • Label slide(s) with patient first and last name (use pencil).
  • Put slide in individual slide holder or Pap-Pak™ and return to the laboratory
  • Include birth date, LMP, last PAP smear, exogenous hormones, drug therapy and any previous pertinent clinical information on the requisition.
Shipping Requirements If mailed, put in padded envelope or box with packing. Maintain slides at room temperature.
Sample Rejection Criteria
  • Name on slide does not match requisition or slide not labeled (name on Pap-Pak™ is not acceptable).
  • Fixation in Formalin
  • Slide severely broken
  • Air dried smear
