Label slide with patient’s first name, last name and DOB (use pencil). |
Special Instructions |
- Indicate specimen source on requisition slip
- Submit 1 smear
- DO NOT allow the smears to dry before applying the fixative provided in the Pap-Pak™ when the sample is collected.
- Label slide(s) with patient first and last name (use pencil).
- Put slide in individual slide holder or Pap-Pak™ and return to the laboratory
- Include birth date, LMP, last PAP smear, exogenous hormones, drug therapy and any previous pertinent clinical information on the requisition.

Shipping Requirements |
If mailed, put in padded envelope or box with packing. Maintain slides at room temperature. |
Sample Rejection Criteria |
- Name on slide does not match requisition or slide not labeled (name on Pap-Pak™ is not acceptable).
- Fixation in Formalin
- Slide severely broken
- Air dried smear
