Once you have completed the form (only the name is required), click the "Submit" button/link and the click on "Copy Signature" button/link to copy it into your clipboard, and then add it to your email's signature creation tool. Microsoft's support site offers a tutorial on how to create signatures in Microsoft Outlook.
