Sanford Assessment

A free, community-based health and wellness assessment for people ages 60 and older, providing a comprehensive review and recommendations. Sanford Assessment will empower you to be your strongest health advocate with:

Comprehensive medication review
Screening for risk of falls
Cognition screening
Support for depression, anxiety, isolation and other social risk factors
Personalized care plan (shared with your primary care provider, if appropriate)
Referrals to appropriate community resources
