Dear Colleagues:

The annual evaluation process is UNR Med's mechanism to identify, build, and sustain faculty members' successes. This annual process brings faculty members and supervisors together to set goals, engage in professional development dialogue, as well as measure and reward success in the context of UNR Med and department missions. The evaluation process has been updated to better reflect the Core Values of UNR Med. All faculty members hired on a continuing appointment (not temporary or letter of appointment) are required to complete an annual self-evaluation.

The 2019 cycle will evaluate activities occurring between January 1 - December 31, 2019 and look ahead to plan activities for the upcoming year, January 1 - December 31, 2020.

Each faculty member is responsible for scheduling a meeting with their department chair, dean or director to review their 2019 self-evaluation and 2020 Role/Goal statements. Be sure that both the supervisor and faculty member sign the first page of the evaluation and the last page of the Role Statement for academic faculty members and Goal Statement for administrative faculty members. Documents must be signed by both the supervisor and faculty member.

Each department/administrative unit must deliver their binder with signed, original documents, as outlined below, to the Office for Faculty, Debra Glogovac, 775-784-6758, dglogovac@med.unr.edu, in the Pennington Medical Education Building, Lower Level, Suite 029, on or before Monday, January 27, 2020, 3:00 p.m.

Chairs, Deans, Directors

Chairs, deans, and directors will provide a preliminary merit rating recommendation, for review and approval by the Dean. Final evaluation and merit ratings will be entered on the evaluation report by the Dean, along with their signature of approval.

Merit Score
    • 3 or 4: Excellent Performance (choose 3 or 4; do not leave blank)
    • 1 or 2: Commendable Performance (choose 1 or 2; do not leave blank)
    • 0: Unsatisfactory or Satisfactory Performance
Merit Rating
    • E = Excellent
    • C = Commendable
    • S = Satisfactory
    • U = Unsatisfactory
Academic Faculty Members

Each academic faculty member will use Digital Measures to generate a 2019 report, as an editable word document. Institutional and Community Engagement Activities should be entered manually into Section C: Service Assignment. A Role Statement for 2020 must be completed, outlining the anticipated percentage effort and planned activities for teaching, research, and service for 2020. Cumulatively, annual role statements help inform decisions about academic promotion.

The UNR Med compensation plan allows for excess productivity supplements. Participation in at least five activities are required to be eligible for payment including: Three activities from the Institutional and Community Engagement Activities list; and two activities from the departmental activities list provided by your department.

Administrative Faculty Members

Administrative faculty members will use the Administrative Faculty Annual Evaluation and Performance Review Form to create their evaluation report. A Goal Statement must be completed to build the foundation for the 2020 evaluation. Administrative faculty do NOT use the evaluation forms on the UNR HR website.

Core Values

Academic faculty members will select a Core Value from within their Role Statement; Administrative faculty members will select a Core Value from within their Goal Statement. These values will be reviewed during the 2020 annual evaluation cycle and new values selected for the next annual evaluation.

Diversity and Inclusion Activities

Effective January 2019, UNR requires each academic and administrative faculty member to set diversity goals. These should be aligned with the goals of the faculty member, the department/unit and UNR Med. Academic faculty members will select a diversity and inclusion activity within their Role Statement; Administrative faculty members will select a diversity and inclusion activity within their Goal Statement.

These goals will be reviewed during the 2020 annual evaluation cycle and new goals selected for review during each subsequent annual evaluation.

Turning in Department Packets

All completed annual evaluations are due to chairs, deans, directors by the date determined by each department/administrative unit. Each department/administrative unit must deliver their binder with signed, original documents, as outlined below, to the Office for Faculty, Debra Glogovac, 775-784-6758, dglogovac@med.unr.edu, in the Pennington Medical Education Building, Lower Level, Suite 029, on or before Monday, January 27, 2020, 3:00 p.m.

Do not staple any of the evaluation materials.

Assembling the Annual Evaluation Binder for Your Department/Unit

Binder Organization

1. Merit Rate Spreadsheet: A completed Excel Merit Spreadsheet should be the first document in the binder

2. Chair/Dean/Director Annual Evaluation Packets: These follow the Merit Spreadsheet and must include:

a. Packet Checklist

b. Annual Evaluation & Merit Review Form

c. Role Statement

d. Current CV: Updated through December 2019

3. Faculty Member Annual Evaluation Packets: These follow the chair/dean/director packet and must include:

a. Packet Checklist: One completed for each faculty member

b. Academic Faculty:

i. Annual Evaluation & Merit Review Form

ii. Role Statement Purpose

iii. Current CV: Updated through December 2019

c. Administrative Faculty:

i. Annual Evaluation & Performance Review Form

ii. Goal Statement

iii. Current CV: Updated through December 2019

Annual evaluation packets should be placed in the provided binder, in alphabetical order (except for the supervisor's packet which is placed in the front of the binder) using tabs created by the department/unit. ALL FORMS MUST BE COMPLETE WITH SIGNATURES.
