Student Record Maintenance

The UNR Med PA Studies Program maintains PA student files documenting academic progress and promotion. The confidentiality and security of education records are of primary importance to the UNR Med PA Studies Program and the university. The university and the PA Program comply with the Family Educational Rights and Privacy Act of 1974 (FERPA) and the University of Nevada, Reno Privacy Notice and Request for Confidential Status of Directory Information.

The program registrar secures all records and is only available to authorized personnel. Students cannot access the records and/or confidential information of other students, faculty, or staff.

Student files include the following sections:

  • Admission and Enrollment Criteria
    • CASPA Full Application
    • Background Check
    • Proof of Health Insurance
    • Proof of Immunization
    • BLS Certification
    • Student Directory Approval
    • Technical Standard Acknowledgment
    • HIPPA Certificate
    • OSHA Certificate
    • Handbook and Policy Acknowledgment
  • Student Performance including Academic Progression
    • Didactic Phase - evaluations and grade reports
    • Clinical Phase - clerkship evaluations and grade reports
    • Commendations
  • Remediation efforts and outcomes
  • Administrative action(s)
    • Academic/behavioral disciplinary action
  • Completion of Graduation Requirements
    • Documentation that students have met requirements for completion

Local current student addresses and telephone numbers must be on file. It is the responsibility of the student to update said directory information regularly in MyNEVADA.

Records Request:

Students are encouraged to review their academic files regularly. Access to a student's file does not include the right to make copies of all or part of the file. Students may request to view their academic files with their advisor or the Dir. of Curriculum. Copies of files will not be allowed unless specifically approved by the Program Director.

Directory Information:

Local current student addresses and telephone numbers must be on file. The student is responsible for updating directory information regularly in MyNEVADA. UNR Med PA Studies Program has several uses for student directory information and a student's likeness/photographic image. Students are advised of their rights under FERPA and should submit the University of Nevada, Reno Privacy Notice and Request for Confidential Status of Directory Information form to the PA Program should they request that the school not disclose directory information and/or likeness/photographic image.

Student Health Records:

The Student Health Center maintains student health records. All services and records are strictly confidential. No health records are accessible to the faculty or staff unless released at the student's request. Any request for records requires a signed release by the patient/student in adherence with HIPAA.
