Cohort based program | 12–1 p.m.
Our aim is to establish a community of practice between primary care providers in Nevada to share knowledge and best practices in managing patients with diabetes. This series will offer health care professionals the opportunity to collaborate and discuss cases with a multidisciplinary team of subject matter experts to enhance diabetes management.

Our objectives are:
- Empower primary care providers to implement Standards of Care for people living with diabetes.
- Collaborate and discuss diabetes cases to enhance diabetes management for the primary care provider.
- Reduce provider isolation in managing diabetes in outpatient and inpatient settings.

Our team
- Karmella Thomas, RD, CDE. Karmella trained at the University of Nevada, Reno, in both biology and nutrition. She has a certificate of Specialized Training in Adult Weight Management from the American Dietetic Association She also earned the Certified Diabetes Educator (CDE) credential in June 2012.
- Heather Townsend, PharmD, BCCCP, BCPS. Heather Townsend earned her Doctor of Pharmacy degree from Idaho State University College of Pharmacy in 2007. She is a board certified pharmacotherapy specialist and board certified critical care pharmacist and was recognized as the pharmacist of the year by the Nevada Society of Health- System Pharmacists in 2016. Heahter has been a Pharmacy Clinical Specialist at Renown Health since 2012 working to develop protocols and improve medication therapy in the emergency department
- Kelsey West, MD. Dr. West is a graduate of UNR School of Medicine and UNR Family Medicine Residency. She currently serves as Associate Professor in the Department of Family Medicine, as well as has her Board Certification in Obesity Medicine. Dr. West enjoys nature, soccer, reading & connecting with the community to promote healthful living and preventive care.
