Specimen Identification Label container with patient’s first name, last name and DOB include date and time of collection.
Special Instructions
  • Indicate specimen source on requisition slip
  • Include any pertinent clinical information or patient history on requisition.
  • Submit two thick and two thin smears if preparing the smear at the clinic office.

  • Call the Micro department for thick smear preparation
